Social Work Case Manager - FastTrack Futures

The social work case manager provides employment support, case management, and outreach to program participants to address barriers and ensure career progress. They also develop partnerships with organizations and facilitate workshops to promote life skill development and relational wellness.

Candidates must have a bachelor's degree in a social science field with up to one year of experience, or four years of experience in a social service setting. Additional requirements include a valid driver's license, the ability to pass background checks, and the capacity to obtain cpr/Aed and first aid certifications.
