Mentor- Santa Rosa

The mentor is responsible for providing face-To-Face support to clients through weekly sessions and home visits. They assist clients with life skills, medication management, and engage in recovery-Related activities while maintaining communication with the case manager.

Candidates must have a valid driver's license and be willing to transport clients as needed. The position is on-Call and hours may fluctuate based on client demand.
