Wrap Facilitator for Lake County

The wrap facilitator helps families develop wraparound teams and creates care plans based on individual strengths and needs. They also facilitate community-Based meetings to review care plans and assist families in accessing necessary services.

Candidates must hold a bachelor's degree and possess at least 24 months of experience in fields such as case management, child welfare, or education. A valid driver's license, reliable transportation, and a designated home office are also required.
