Service Coordinator-Bilingual

The service coordinator facilitates the development and evaluation of individual family service plans (Ifsp) and maintains regular contact with families to ensure timely service delivery. They also coordinate with health care providers, assist with community resource navigation, and ensure all case files remain audit-Compliant.

Candidates must hold a bachelor's degree in social work, human services, or child development and possess at least one year of experience working with clients with special needs. Fluency in spanish and a valid wisconsin driver's license with a clean driving record are mandatory requirements.
