Position Summary

Supports adults with disabilities in achieving their vocational goals by developing person centered plans along with coordinating, implementing and facilitating activities to include, managing the development, maintenance and communication of mandatory tracking and reporting of progress to goals. Serves as a liaison to community partnerships.

Qualifications

Minimum Education, Training & Experience: High school diploma plus three years of experience of working with adults with developmental and/or cognitive disabilities Or Associate’s degree in human services, education, psychology or related field plus one year of experience of working with adults with developmental and/or cognitive disabilities Preferred Education, Training & Experience: Bachelor's degree preferred ESSENTIAL SKILLS AND ABILITIES: Ability to be positive and outgoing and demonstrate strong interpersonal skills. Ability to assist the people we support to pursue and obtain their personal goals. Ability to effectively communicate orally, in writing, and on the telephone. Strong organizational skills Ability to work effectively without direct supervision to ensure completion of mandatory daily and monthly documentation. Ability to work cooperatively with all departments, levels of staff, and outside contacts. Ability to work in varying climate/element/locations (sites are based in the community and may include both indoor and outdoor activities). Ability to effectively balance multiple tasks and ever-changing priorities. Ability to assist the People We Support (PWS) with ADL’s (Activities of Daily living) and ILS (Independent Living Skills).
