Core responsibilities
The transitional facilitator will provide case management, life skills training, and employment assistance to young adults involved with dmh. They are responsible for coordinating intake assessments, monitoring participant progress, and maintaining a network of community support services.
Requirements summary
Candidates should have at least one year of experience working with adolescents or young adults in residential or dmh-Related programs. A bachelor's degree is preferred, and a valid driver's license with access to a reliable vehicle is required.
