School Based Community Case Manager

The case manager will provide clinical and administrative support to children with mental illness within a school-Based setting. Responsibilities include managing patient treatment plans, transporting patients, and participating in community education.

Candidates must possess either a bachelor's degree or four years of equivalent work experience. A valid driver's license, the ability to pass extensive background checks, and experience working with children are required.
