Full-time 35 hours per week Hybrid schedule may be possible after six months. Assesses the ability to independently coordinate tasks associated with the Personal Care Management (PCM) program, assists consumers in developing skills and resources to manage PCA services and activities of daily living. Requires good assessment skills, home visits, knowledge of community services, good oral/written communication, data entry skills. Travel reimbursement is $0.62/mile
Written and Verbal Fluency in English and Spanish is required. High School Diploma or equivalent is required. Bachelor's degree is preferred. Must have valid driver's license and reliable transportation. Must be able to climb multiple flights of stairs. Public Service Loan Forgiveness (PSLF) If you are employed by a not-for-profit organization such as GSSSI, you may be eligible for the PSLF Program for student loans.
