Transition Coach

The transition coach acts as an advocate and guide for young adults in residential programs to help them achieve goals and transition to independence. Responsibilities include developing individual treatment plans, providing therapeutic coaching, and ensuring a safe, structured environment.

Candidates must be at least 21 years old with a bachelor's degree in behavioral or social science and a valid illinois driver's license. Preferred candidates have at least two years of experience working with youth who have experienced trauma or developmental adversity.
