Supports Coordinator 1

Develop and review person-Centered plans for participants while coordinating necessary supports and community resources. Monitor progress, conduct interventions, and maintain compliance with carf, medicaid, and department of mental health standards.

Requires a bachelor's degree in social work, psychology, or a related field and one year of experience with individuals with intellectual or developmental disabilities. Must possess a valid michigan driver's license and obtain michicans certification within 90 days.
