What We Offer

Public Service Loan Forgiveness Qualifying Employee What You’ll Do Support Members in Reaching Their Career Goals Link members with collaborative employer relationships to achieve competitive employment goals. Assist members with benefits counseling (e.g., SSI, Medicaid impact), reporting earnings, and navigating disclosure of psychiatric status to employers. Provide On-the-Job Coaching and Case Management Deliver individualized job coaching and ongoing follow-along supports to help members succeed in their roles. Develops an individual employment (and/or education) plan with members. Offer key education and support for employers, family members, and other stakeholders (with member permission). Maintain Accurate Documentation and Partner Communication Returns phone calls and reacts to situations in a timely manner. Work closely with the Program Director to keep participant records up to date. Coordinate monthly, in-person meetings with DVR counselors and submit required monthly reports. Engage the Community and Promote the Program Conduct proactive outreach to members who have disengaged to support re-engagement. Build and maintain strong relationships with local employers and community partners. Represent the Program Director or designee in internal and external meetings as needed. Manage Transportation and Additional Supports Transport members as needed using a personal vehicle that meets safety requirements. Provide supported education services for members pursuing academic or skills training. Perform additional duties as assigned by the Program Director or designated supervisor.

Qualifications

Associates degree in a Human Services or equivalent preferred. Certified Employment Support Professional (CESP) certification, or ability to obtain within 30 days of hire required. Qualified Medication Administration Personnel Certification (QMAP), or ability to obtain within 30 days of hire. CPR and First Aid Certifications, or ability to obtain within 30 days of hire. Safety Care certification, or ability to obtain within 90 days of hire. Demonstrated professionalism, empathy and respect when interacting with diverse individuals and navigating complex situations. Ability to maintain an unwavering positive attitude and resilience in the face of challenges. Strong ethical standards, including honesty, self-discipline, and personal accountability. Effective and respectful communication in both verbal and written formats, adapting to individual and group settings. Proficient computer skills and digital literacy. Reliable, supportive, and collaborative team member. Ability to work an open and flexible schedule to meet assigned member and program needs, including some holidays, nights and weekends. Ability to work with sensitivity to the cultural and/or socioeconomic diversity of the agency’s clients and staff. Ability to maintain a high level of confidentiality and trust. Must successfully complete a comprehensive background screening as a condition of employment, in accordance with Colorado Department of Human Services regulations for childcare facilities. This screening includes initial and ongoing state criminal background checks, fingerprinting, sex offender registry searches, and abuse and neglect clearances. If the employee has lived in any other state within the past five years, equivalent background and abuse/neglect checks from those states will also be conducted. In addition, employees are required to complete a health evaluation or physical examination prior to beginning service to confirm fitness for duty, and annually thereafter Must have a valid driver's license (with acceptable DMV record), and a willingness to transport clients in your vehicle Must be at least 21 years of age.
