Job description
About the Department
- Assess and treat patients referred by school staff, parents, PCP offices, and self-referred patients.
- Provide individualized therapy services and monitor patient progress.
- Develop individual plan of care and give recommendations to school staff, parents, PCPs, specialists, and other members of the interdisciplinary care team regarding mental health treatment strategies and identifies approaches to maximize continuity of care across the continuum.
- Respond to urgent mental health concerns, including suicide risk assessments and assist in connecting patient to appropriate level of care based on clinical assessment.
- Assist with greater Ambulatory Care Behavioral Health teams work as assigned.
- Participate in professional development opportunities through peer case review and monthly continuing education opportunities.
- Other job duties as assigned.
