  • As a Community-Based Service Provider (CBSP), you will deliver direct services to children and their caregivers enrolled in Children & Family Treatment Supports and Services (CFTSS) and Home & Community Based Services (HCBS) programs.
  • Using a person-centered planning approach, you will engage youth and caregivers to develop personalized treatment plans and identify future goals.

Responsibilities

Provide goal-driven, direct services to assigned youth and caregivers. Develop and implement treatment plans, safety plans, and service-related documentation. Collaborate with other providers to coordinate services that support the child's and family's overall health and wellbeing. Track and monitor service hours for a diverse caseload. Work within a multi-disciplinary team and ensure services align with treatment goals.

Requirements

Bachelor's Degree in Human Services or related field. Two years of experience working with children to support their development and wellbeing. Knowledge of mental health diagnoses, symptomatic behaviors, and trauma responses. Flexible schedule (including evenings and weekends). Bilingual (English-Spanish) preferred. Strong communication (oral and written) and technology skills. Effective engagement skills; creative and resourceful.
