Responsibilities

Maintain a caseload of approximately 15 providers and their associated families Conduct initial and ongoing assessments of family needs Develop individualized work plans that include goal setting, home visits, and family support services Collaborate with educational staff to design monthly parent engagement activities Provide referrals and resources tailored to each family's needs Support network providers through site visits and ongoing communication Promote healthy family development through education and consistent engagement

Requirements
