David Raines Community Health Centers is looking for passionate, qualified applicants to join our team!
The School-Based Health Center (SBHC) Pediatric Nurse Practitioner (PNP) provides comprehensive primary care, acute illness treatment, chronic condition management and screenings to students in a school setting. The PNP will play a crucial role in providing comprehensive healthcare services to children in the Shreveport/Bossier.
Work Experience Requirements
- Perform well-child exams and sports physicals.
- Administer vaccinations and other preventive healthcare measures.
- Collaborate with pediatrician and other healthcare professionals to develop and implement care plans.
- Monitor and manage patient progress, adjusting care plans as needed.
- Diagnose and treat acute illnesses and manage chronic conditions.
- Screen, evaluate and provide referrals regarding health issues.
- Adheres to the standards of medical practice.
- Provide education to patients and families regarding nutrition and wellness.
- Coordinate care with school administration and patients.
- Maintain accurate patient records via electronic health records.
- Performs other duties is described.
Education/Experience Requirements
- Master of Science Degree in Nursing with concentration in Nurse Practitioner required.
- Minimum 2 years of experience in pediatric healthcare setting preferred.
- Experience with electronic health records required, preferably Next Gen or similar EHS.
- Strong clinical judgment and decision-making skills.
- Excellent interpersonal and communication skills.
- Ability to work effectively both independently as part of a multidisciplinary team.
- Must be available to work after hours and weekends as required by management.
Licensure/Certifications
- Must be licensed as a Nurse Practitioner to practice in the State of Louisiana.
- Board certification as a Pediatric Nurse Practitioner (PNP or FNP – Peds).
- Current BLS (American Heart Association).
- Active DEA licensure
