Job description

  • JOB RESPONSIBILITIES Coordination and Preparation - Reports educational progress to patient's school.
  • Collaborates with intradisciplinary team on preparation for transition back to school.
  • Consults with patient and family regarding plans for re-entry.
  • Facilitates the re-entry process following discharge.
  • Participates in patient care conferences.
  • Interaction - Meets new patients and gathers pertinent information through observation and interview.
  • Assesses educational needs in classroom and at bedside.
  • Makes appropriate recommendations based on accurate assessments.
  • Makes internal and external referrals as indicated.
  • Assessment - Assesses educational level and cognitive status of patients.
  • Identifies physical readiness and cognitive readiness for participation in school program.
  • Goal Setting - Formulates appropriate educational goals.
  • Develops curriculum based on individual patient educational goals.
  • Implements educational programming based on educational assessment to maximize growth and development that is age/grade level appropriate.
