Implement the HealthySteps model to support the development and wellbeing of children aged 0-5 and their families. Coordinate care through in-person well visits, community referrals, and clinical documentation within a pediatric primary care team.
Requires a Master's degree in social work or a related field and 1-3 years of experience working with children and families. NYS Licensure and completion of HealthySteps Specialist training are required within six months of hire.
