Registered Nurse
- Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities.
- Monitors patients, administers medications, keep records, consult healthcare providers, educates patients and more.
- Assess patient knowledge and provides education in group and individual settings for patients with Type 1, Type 2, Gestational Diabetes, and Pre-Diabetes.
- Provides education tailored to special learning needs.
- Coordinates patient care and adjustments in care plans in coordination with clinicians.
Principal Accountabilities

- Coordinates individualized patient and support system assessment, education, planning and implementation of care goals.
- Collaborates with the referring clinicians and interdisciplinary team to work toward optimal patient education outcomes and glycemic control.
- Maintains professional education standards required to be a CDCES in an ADA approved Diabetes Self- Management Education program.
- Engages in research and quality improvement activities.
- Other duties as assigned.
