Provide family-centered mental health counseling, crisis intervention, and case management for youth in homes, schools, and community settings. Collaborate with multidisciplinary teams to support stability and coordinate care through treatment reviews and IEP meetings.
Requires a high school diploma with 2 years experience, an associate degree with 1 year experience, or a bachelor's degree. Must be at least 21 years old with a valid driver's license, own vehicle, and necessary background clearances.
The Community-Based Counselor provides family-centered mental health counseling, support, crisis intervention, and care coordination services for youth and their families. As part of a multidisciplinary team, Community-Based Counselors deliver trauma-informed, culturally responsive services in homes, schools, and community settings, helping youth and families build stability, strengthen relationships, and address challenges that may impact their well-being and daily functioning.
About the Program
The San Francisco Wraparound Program partners with the Department of Public Health to support youth and families experiencing significant behavioral, emotional, and mental health challenges. Serving youth who are at high risk for hospitalization, school disruption, placement instability, and other adverse life outcomes, the program provides intensive, strengths-based, and individualized services. Through a collaborative team approach, the Wraparound Program works alongside youth, families, and community supports to build on existing strengths, address unmet needs, and promote long-term stability and success in the home, school, and community.
