The Home Visitor provides comprehensive, long-term home-visiting services to high-risk first-time parents and families with children up to 5 years old. Responsibilities include conducting assessments, developing family goals, and facilitating connections to community health and mental health services.
Candidates must have an Associate's degree in Social Work or Child Development (Bachelor's preferred) and at least one year of experience working with young children. A valid driver's license, insurance, and a reliable vehicle are required for in-person visits.

The Home Visitor provides comprehensive, long-term home-visiting services within a strength-based, family-centered model to high risk first-time parents and families with young children, birth through 5 years of age. The Home Visitor also assesses infant/child and family needs, help families to establish plans to reach family and child development goals, assists families to expand their support systems, provides parenting support, and facilitates groups.
