Provide individualized diabetes self-management education and training to patients using the AADE7 framework. Develop educational goals and follow-up plans while collaborating with a multidisciplinary team to improve patient outcomes.
Requires a Bachelor's degree and professional licensure as an RN, RD, CSW, or Pharmacist. Must be a Certified Diabetes Educator with at least 6 months of clinical experience.
Provides individualized diabetes self-management education / training to individuals and groups according to the Competencies for Diabetes Educators and ADCES Diabetes Educator Practice Level 3. Able to communicate effectively with patients of all ages and abilities. Demonstrate diabetes self-care management skills to persons with type 1, type 2, gestational and pre-diabetes.
Collects DSME program participant assessment data, using the AADE7 framework, in a collaborative and ongoing manner. Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME program participants. Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods. Collaboratively develops an individualized follow –up plan with each program participant. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives Conducts a follow-up assessment upon completion of DSME program services, using outcome measures form the AADE7. Document assessment data, educational plan, educational services provided and evaluation of result in each participant's educational record. Utilizes a team approach to provide services, collaborates, and communicates with team members when needed. Identifies when a program participant's needs are outside the scope of the instructor's practice and expertise, and arranges for additional services to meet needs. Participates in the development of training materials. Contributes to, and participates in, a continuous quality improvement process to measure DSME program and to identify and address opportunities for improvement. Performs other duties as assigned.
Bachelors required, Masters preferred Graduate of an approved program as RN, RD, CSW or Pharmacist. Current professional licensure, registration and certification. Minimum of 6 months clinical experience as Certified Diabetes Educator. Certification in insulin pumps preferred. Pay Range: $35.75- $47.78 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
