$130.00 per hour based on skills and experience

Position Summary

The Community Care Connection (CCC) PRN Physician is responsible for providing high quality, patient-centered medical care and health education through the Community Care Connection mobile clinic (Care Coach) as part of the Rosalind Franklin University Health Clinic (RFUHC). This role supports the delivery of accessible healthcare services to underserved and vulnerable populations within the community.

The PRN Physician conducts medical consultations, performs comprehensive patient assessments, diagnoses and treats acute and chronic health conditions, and develops appropriate care plans. The physician provides direct patient care, prescribes medications and treatments in accordance with applicable laws and clinical guidelines, and delivers preventive health education to promote wellness and disease management. Additionally, the physician collaborates with interdisciplinary healthcare teams, community partners, and learners to ensure continuity of care, facilitate referrals, and support the mission of improving community health outcomes through compassionate, evidence-based care.

This position requires a commitment to patient advocacy, health equity, and the provision of culturally responsive care in a mobile and community-based healthcare setting.

While this position is not eligible for our regular employee benefits, all RFU employees are eligible to accrue personal leave in accordance with IL law. All RFU employees are also eligible to make unmatched contributions to voluntary supplemental pre-tax and post-tax retirement plans.

  • Essential Duties & Responsibilities
  • Provide comprehensive primary and preventive medical care to patients served through the Community Care Connection (CCC) mobile clinic in accordance with accepted standards of medical practice.
  • Conduct patient assessments, obtain medical histories, perform physical examinations, and develop appropriate diagnoses and treatment plans.
  • Prescribe medications, therapies, and other interventions in accordance with federal and state regulations, organizational policies, and clinical best practices.
  • Deliver patient-centered care that is culturally responsive, trauma-informed, and sensitive to the needs of underserved and vulnerable populations.
  • Provide health education, disease prevention counseling, and self-management support to patients and caregivers.
  • Order, interpret, and communicate laboratory, diagnostic and screening test results as appropriate.
  • Collaborate with nurses, advanced practice providers, pharmacists, behavioral health professionals, community health workers, and other members of the interdisciplinary care team to coordinate patient care.
  • Facilitate referrals to specialty care, community resources, and social service organizations to support continuity of care and address social determinants of health.
  • Maintain accurate, timely, and complete patient documentation within the electronic health record (EHR) system in compliance with regulatory and organizational requirements.
  • Participate in quality improvement initiatives, patient safety activities, and performance improvement efforts to enhance clinical outcomes and service delivery.
  • Support the educational mission of Rosalind Franklin University by serving as a clinical role model and collaborating with students, residents, and other learners as appropriate.
  • Adhere to all applicable federal, state, and local regulations, professional standards, accreditation requirements, and RFUHC policies and procedures.
  • Participate in mobile clinic operations, including pre-clinic preparation, patient flow management, and post-clinic follow-up activities as needed.
  • Assist in identifying community health needs and contribute to outreach efforts that promote access to care and health equity.
  • Maintain current licensure, certifications, credentialing, and continuing medical education requirements necessary to fulfill the responsibilities of the position.
  • Perform other duties as assigned to support the mission, goals, and operations of the Community Care Connection and Rosalind Franklin University Health Clinic.
  • Additional PRN-Specific Expectations
  • Accept and fulfill scheduled PRN shifts based on program needs and physician availability.
  • Demonstrate flexibility in providing care across various community-based locations served by the mobile clinic.
  • Communicate availability and scheduling changes in a timely manner to ensure continuity of clinic operations.
  • Remain current on mobile clinic protocols, workflows, and community partnerships to support efficient service delivery.

Conditions of Employment

  • Must achieve satisfactory results from a background check.
  • Must have proof of COVID 19 vaccination with booster or a university-approved exemption.
  • Required Education & Experience
  • Graduation from accredited medical school and residency required.
  • Completion of Fellowship programs (if applicable) also required.
  • Successful candidates must:
  • Possess an unlimited license to practice medicine in the State of Illinois;
  • Possess an unrestricted right to prescribe controlled substances and medication;
  • Be board certified, eligible or qualified in their specialty.
