The Care Coordinator manages the overall coordination of individual care for youth in a residential treatment center, collaborating with clinical, medical, and administrative staff. Key duties include handling admissions, discharge planning, family outreach, and maintaining detailed clinical documentation.
Candidates must be at least 21 years old with a high school diploma and 4 years of experience, or a bachelor's degree and 1 year of experience in mental health or social services. A valid Arizona Driver's License and a Fingerprint Clearance Card are required.
Pay Range: $20.00 - $22.00/hr. Must be 21 years of age or older High School Diploma or GED required; Bachelor’s degree in Human Services, Psychology, Social Work or relevant field preferred Four (4) years related experience in mental health/social services field with high school diploma/GED OR one (1) year of related experience with Bachelor’s degree Must demonstrate excellent verbal and written communication skills Ability to handle multiple tasks and meet deadlines with minimum supervision; self-motivated General knowledge of clinical best practices for working with adolescent populations and their families/resources General knowledge of mental health, funding and related services within Arizona and other states served. General knowledge of standard office computer applications. OTHER: Must be 21 years of age or older Drug test within 24hrs of verbally accepting offer w/ negative results Must be available to attend 2 weeks of required orientation training (paid, Mon-Fri 8:30am-5:00pm) plus additional 2-3 weeks on the job training Valid Arizona Driver's License & pass DMV 36-month history check Pre-employment Physical and Job Demand Assessment (if applicable) Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions).
