Sacred Circle Healthcare is seeking a full-time, Family Spirit Home Visitor, in Salt Lake City, Utah.
The home visitor position will be responsible for providing comprehensive, individualized home visits to families, utilizing the Family Spirit curriculum to teach lessons to parents and children from prenatal to 5 years old. This role will focus on enhancing child development, supporting positive parenting, strengthening family capacity and connecting families to community resources as needed. This position requires a caring individual who is committed to fostering positive growth and development within families. The ideal candidate is comfortable working independently, has strong organizational and data management skills, is self-motivated and adaptable, has a warm and engaging personality and enjoys providing caring support to parents and children. Prior experience working with children and/or in a clinical setting required.

Benefits: Eligible:
Hours Per Week: 40
45 am – 7:00 pm Monday through Friday. Typically, five 8- hour shifts per week. Evening and weekend work may be required depending on position requirements.
Training: Complete virtual core training for the Family Spirit program in August 2026. Attend in-person enhanced Family Spirit training in Albuquerque, NM in November 2026. Travel expenses are employer-paid. Attend on-going Family Spirit and DHHS trainings as required.
Individualized
Home
Visits: Using the Family Spirit lesson plans, the home visitor plans, develops, and implements individualized home visits for families on their caseload, either at the family’s home, the clinic, or another approved location.
Client
Referrals: Follow up with referrals of potential clients to inform them about the Family Spirit program and schedule intake appointments. Generate referrals as needed through community outreach.
Documentation
Management: Monitors caseload to ensure all required documentation and requirements are completed on time. Ensure all enrollment and intake paperwork, consents, screenings, assessments, visits, etc. are documented appropriately within the required timeframes. Ability to use online documentation system in English.
Community
Engagement: Assists with recruitment events in the community to educate about the Family Spirit program and its availability.
Grant
Compliance: Meet all grant requirements including documentation and data gathering for visits.
Event
Planning: Assists with planning and conducting community activities and events.
Screening and
Assessments: Complete ASQ training and conduct regularly scheduled screenings and assessments, along with the required documentation.
Family
Support: Assists families with referrals to community agencies to fulfill their needs.
Travel
Requirements: Travels throughout Salt Lake County using the company vehicle (preferred) or your personal vehicle (mileage reimbursement provided). Some travel to Wendover and Ibapah in the company vehicle may also be required.
Team
Duties: Attends weekly team meetings, assists with management of supply inventory, submits monthly team reports and helps care for the office and program vehicle as needed.
Reads, understands, and adheres to all Sacred Circle Health Care Policies and Procedures.
Practices and adheres to the Code of Conduct and Mission and Values Statements.
Other Duties as Assigned.
High school diploma or GED equivalent.
One (1) year of experience providing direct support, education, outreach, or services to patients, clients, children, or families in healthcare, community health, social services, early childhood, home visiting, or related setting.
Valid driver’s license.
CPR Certification (provided upon hire).
English language fluency.
Experience working with the Native American community.
Degree in child development and/or early childhood education.
Three (3) years of experience working with children and families.
LICENSE: Medical or related certification
Knowledge, Skills, and Abilities
Strong understanding of child development and early childhood education.
Availability to attend required trainings.
Excellent communication and organizational skills.
Compassionate and empathetic approach to working with families.
Ability to work independently and manage multiple tasks.
Ability to take directions and respond positively to feedback in the workplace.
