Tresco, Inc. is seeking a compassionate, organized, and driven Family Service Coordinator to lead and support our Home Visiting professionals. This role is essential in providing quality enrollment, eligibility services, and empowering families with the resources they need to thrive.

Coordinate accurate intakes, evaluations, and assessments to determine eligibility Facilitate and actively participate in the development and reviews of Individual Family Service Plans (IFSPs) Support families in accessing community resources beyond the child’s condition Coordinate services and monitor delivery, including subcontractors and cross-agency providers Maintain up-to-date and accurate child files in compliance with accreditation and Medicaid standards Facilitate transition planning for children exiting the program Gather and manage a community resource register to better serve families Stay current through required training programs and professional development
