Core responsibilities
The case manager is responsible for scheduling intake meetings, completing required documentation, and developing individualized employment plans in collaboration with participants and support teams. This role also involves coordinating onboarding paperwork, tracking participant progress toward employment goals, and participating in planning meetings for various employment services.
Requirements summary
Qualified candidates must possess a bachelor's degree in special education, vocational rehabilitation, or a related human services field, along with knowledge of employment services and person-Centered planning. Strong organizational, communication, documentation skills, and computer proficiency are required, as is the ability to maintain confidentiality.
