Community Habilitation Specialist - Brooklyn

The community habilitation specialist provides ongoing training and personal assistance to participants with disabilities to promote independence and community inclusion. They work with minimal supervision, following individualized habilitation plans and supporting participants in various activities.

Candidates must have a high school diploma and preferably some college experience working with people with disabilities. They should also be able to read, write, and speak english, and possess computer literacy for electronic medical record software.
