Case Manager (HFA Bilingual Advocate)

The bilingual hfa case manager will provide weekly in-Home visits to families facing various challenges and support their well-Being through evidence-Based education. They will also create family service plans and represent mcc at community events.

Candidates must have a degree in social work or a related field and be bilingual in english and either spanish or haitian creole. They should also be willing to work non-Traditional hours and have a valid u.s. Driver's license.
