Family Health Navigator

The family health navigator will improve health and birth outcomes for pregnant or parenting individuals by linking them with community resources and facilitating integrated service delivery. The role involves conducting home visits, engaging in community outreach, and providing health education to promote positive health behavior changes.

A high school diploma or ged is required, with a preference for a bachelor's degree. Candidates should have at least one year of experience working with diverse families, or relevant lived or volunteer experience.
