Core responsibilities

The day program case manager will build and foster professional relationships with individuals with developmental disabilities and their interdisciplinary teams. They will ensure ongoing coordination of services and monitor the effectiveness of support plans.

Requirements summary

A high school diploma is required, with a preference for a ba degree in human services or a related field. At least one year of experience supporting adults with developmental disabilities is preferred.
