Education Transition Specialist

The specialist provides transitional independent living resources to case managers for youth aged 16 and over in foster care, which includes scheduling and hosting transition meetings and explaining dcf benefits at case planning conferences. Key duties involve assembling transition packets, locating scholarship opportunities, assisting with post-Secondary education applications, and acting as a liaison between schools and case management teams.

A minimum requirement is a high school diploma and a valid driver's license with auto insurance, along with intermediate computer skills in microsoft word, excel, and outlook. Previous office experience and experience working with high-Risk or foster care children are preferred qualifications.
