CSF Parent Navigator

The parent navigator provides direct support and service coordination to families in region 5, utilizing strength-Based and culturally responsive practices to develop individualized engagement plans. Responsibilities include meeting with families, maintaining contact, assisting with the plan of care development using the wrap process, and connecting families with community resources.

Candidates need an associate's or bachelor's degree in a human services field, or an unrelated degree with at least three years of relevant family work experience and strong community resource knowledge. A valid connecticut driver's license, reliable personal vehicle for home visits, and cpr/First aid certification are mandatory requirements.
