- 1300Lehigh - Philadelphia, PA 19132The Permanency Contractor is responsible for coordinating all activities related to tracking and implementing tasks related to SWAN clients meeting specified permanency goals and Emergency Kinship/Prospective Parent initial home certification.The Permanency Contractor will work closely with Administrative, Supervisory and Case Management Staff to assure that permanency goals are achieved.The Permanency Contractor will demonstrate an understanding of all the contracting agencies (County) performance standards governing the provision of services and the agencys own standards for governing service provision.

- RESPONSIBILITIES: (and other duties as assigned) 1.Developing Family Profiles, Initial Certification Home Study, Child Profiles, Child Preparation Services, and Finalizations 2.Participation in the development and implementation of Child Specific Recruitment, Matching, and Post-Permanency plans 3.Maintaining an organized filing system of all cases 4.Provision and coordination of case management services for families receiving after care services as well as for all new client admissions 5.Knowledge of community-based resources for children and families in Philadelphia 6.Maintaining a strong connection with affiliate agencies/County through trainings and other functions 7.Knowledge of child welfare system in Philadelphia 8.Coordination of agency permanency activities with County and Affiliate Agencies 9.Knowledge of SWAN and DPW Performance Standards, State licensing and regulations 10.Representing the agency in court during permanency hearings and at County during permanency roundtables 11.Updating Profiles as required by County Agency 12.Other duties as assigned QualificationsMINIMUM QUALIFICATION Bachelors degree plus 2 years experience working with children and adults Strong writing skills Excellent interpersonal and organizational skills Valid Pennsylvania Driver License Automobile with current registration, insurance and inspection Willingness to work flexible hours Willingness to travel to resource parent homes Ability ascend flights of stairs Ability to speak English clearly and hear voice of conversational tones
