Core responsibilities
The coordinator assists public school students with intellectual developmental disability diagnoses and co-Occurring complex mental health or substance use issues by navigating behavioral health, physical health, and school systems using an integrated whole-Person approach. Key duties involve planning and care management, including utilizing person-Centered planning, collaborating with various stakeholders, assessing sdoh, and ensuring appropriate services are implemented and monitored.
Requirements summary
Candidates must possess a bachelor's degree in a human service field with two years of post-Bachelor's experience with the idd population, or a non-Human service degree with four years of relevant experience. Required abilities include exceptional interpersonal and communication skills, strong problem-Solving capabilities, and knowledge of behavioral health, idd, and relevant social service/Court systems.
