Case Manager V - Certified Peer Training and Services - Hamiliton Unit (030019)

This role involves performing highly advanced peer support and case management, coordinating program activities, facilitating interactions between specialists and persons served, and leading educational panels. Essential functions include providing peer skills training, monitoring recovery groups, compiling participant records, and coordinating transition planning to ensure continuity of care.

Candidates must have a high school diploma or ged, plus five years of relevant experience in advocacy, mentoring, education, counseling, or case management, with college credit potentially substituting for some experience. A current, valid certification from the texas certification board as a peer recovery support specialist (Or equivalent) is required, with a supervisor certification being preferred.
