Core responsibilities
The coordinator is responsible for completing all related individual support plan (Isp) paperwork, maintaining confidential records, attending isp meetings, and maintaining associated data systems. Essential duties also include preparing annual isp update packets, completing progress notes, and training/Supervising staff on isp goals and data collection.
Requirements summary
Candidates must possess a bachelor's degree or equivalent experience in human services, along with three years of experience with the dds isp system, preferably including hcsis experience. Applicants must also have acceptable background checks, the ability to drive 16-Passenger vans, a valid driver's license with satisfactory record, and must obtain and maintain various mandatory certifications.
