Community Health Worker - School Health

The community health worker will meet with individuals and families to conduct needs assessments, identify socio-Economic issues affecting health, and connect them with appropriate community and organizational resources. This role involves coaching patients in goal development and self-Care management, coordinating care plans, and facilitating community health education events.

Candidates must complete required training, including the community health worker education program, within one year of employment and possess a valid driver's license with minimal moving violations. Preferred qualifications include experience in public health, social work, or related fields, and bilingual skills are a plus.
