Case Planner

The social worker/Case planner is responsible for comprehensive service coordination for residents, including planning services and monitoring their implementation. They also facilitate family communication and arrange transition services for youth nearing discharge.

Candidates must have at least a bachelor's degree with experience in child welfare, and a master's degree is preferred. A minimum of 2 years of relevant experience and knowledge of fasps and court reports is required.
