ThriveKids Care Coordinator (LMSW/LCSW)

The care coordinator will provide care coordination services to youth enrolled in the thrivekids program and serve as a liaison between medical providers, families, and school personnel. They will also facilitate school re-Entry meetings for students following extended absences and contribute to behavioral health program development.

Candidates must hold a master's degree in social work and be a licensed lmsw or lcsw in the state of louisiana. Experience or training in providing services to children and families is required, along with strong communication and clinical assessment skills.
