Clinical Case Manager I

The clinical case manager conducts developmental evaluations, family assessments, and provides direct services as part of a multidisciplinary team. They are responsible for maintaining accurate client records, coordinating community referrals, and ensuring compliance with state and federal service delivery timelines.

Candidates must hold a master’s degree in an appropriate field or a bachelor’s degree with two years of relevant experience. Additional requirements include a valid driver’s license, eligibility for a medicaid provider number, and successful completion of a level ii-Dcf criminal background screening.
