Position: Transition Specialist

Pay

$21 - $23/hour

Location: Buffalo, NY

Schedule

  • Monday –
  • Friday, 8:00 AM – 3:30 PM, 37.5 hrs/week

Your Impact as a Transition Specialist

  • The Transition Specialist conducts and monitors implementation of transition services on a student’s IEP, such as training, education, employment, and where appropriate, leisure and independent living skills. This may include but is not limited to direct assistance in collaboration with the multi-disciplinary team for persons in supported employment placements, other job experiences in addition to in-house work and the school store, and other community integration activities for the purpose of increasing independence and to foster a successful transition into the adult world.
  • Collaborate with the multi-disciplinary team for persons in supported employment placements, other job experiences in addition to in-house work and the school store, and other community integration activities for the purpose of implementing plans for increasing independence and to foster a successful transition into the adult world
  • Maintain regular and professional communication with classroom teams, related services, behavior support staff, administration, and employers at work site in relation to assigned students
  • Supervise and ensure completion of jobs for in-house job placements (e.g. school store, recycling, and completion of other jobs from outside employers) including the creation of vocational-related materials and maintenance of vocational spaces.
  • Collect and maintain assigned data/tracking, complete assigned assessments and progress updates, maintain record of student training hours, and support the completion of workplace documentation.
  • Maintain CDL license to ensure safely transporting students to and from work placements

  • Adheres to the Summit Center employment policies as outlined in the Employee Handbook including, but not limited to dress code, attendance and punctuality, and Paid Time Off

EDUCATION and/or EXPERIENCE

  • High school diploma and 2 years experience working in the field of developmental disabilities OR
  • Associates degree or higher and 1 year experience.
  • Retail management experience preferred.

Certificates, licenses, registrations etc.

  • NYS Teaching Assistant Certificate or valid NYS Teacher Certificate in any field required.
  • Must be First Aid and SCIP-R certified (training provided by agency).
  • Most hold a valid NYS commercial driver’s license, or complete CDL training within one year of job acceptance. Agency approval of driving record according to agency policy and procedures.
