  • The CHW supports individuals and families by providing health education, connecting them with resources, promoting wellness, and helping reduce barriers to care.
  • Supervisory Accountability: None NATURE AND SCOPE: Community settings, clinics, nonprofit organizations, or public health departments.
  • May include outreach events, and occasional evenings/weekends Responsible for advocacy and problem-solving, health education and communication, and collaboration and teamwork PRINCIPAL ACCOUNTABILITIES: Provide culturally appropriate health education and outreach Connect clients with medical, behavioral health, and social service resources Assist individuals with care coordination and follow-up appointments Conduct community outreach Help clients navigate healthcare systems and insurance processes Advocate for community needs and support health equity initiatives Document client interactions and maintain accurate records Participate in case conferences and interdisciplinary team meetings Support chronic disease management Basic computer skills in operating systems, internet & email proficiency, Word, Excel, file management, create documents, organize files and solve simple problems related to the computer and smart phone technology CORE COMPETENCIES: Knowledge of community resources and local populations Strong communication and interpersonal skills Ability to build trust with diverse communities Basic computer and documentation skills MINIMUM REQUIREMENTS: High school diploma or GED (associate or bachelor’s degree in public health, social work, or related field preferred) Compliance
