Case Manager, Transitional Program & Alumni R

The case manager provides individualized support, resource coordination, and goal-Oriented planning for current and former program participants. They also oversee the follow-On program, manage scholarship applications, and maintain databases to track participant outcomes.

Candidates must hold a bachelor's degree in social science or possess comparable experience in youth residential settings. The role requires proficiency in data management, crisis intervention, and the ability to pass background checks and maintain relevant certifications like cpr and first aid.
