Medical Case Manager III

The medical case manager coordinates care and provides health education to patients living with or at risk for hiv. They also conduct community outreach and lead workforce development training while collaborating with the ryan white and prep teams.

The position requires a bachelor's degree and the ability to work effectively within a multidisciplinary team. Candidates must be capable of linking patients to supportive services and providing ongoing education and communication support.
