Student Success Coordinator

The student success coordinator provides care coordination and psycho-Educational interventions for a caseload of 15 students within the mindcraft program. They act as a facilitator and advocate, developing comprehensive care plans and collaborating with families and school staff to improve student outcomes.

Candidates must hold a bachelor's degree in social work, psychology, or a related field with at least 2-3 years of relevant experience. Proficiency in electronic health records, microsoft office, and knowledge of community resources in montgomery county, maryland, is required.
