The Foster Care Case Worker plays a vital role in our mission to deliver coordinated, individualized care to our communities. As a Foster Care Case Worker, you will provide: Case Management and Support: Conduct interviews with children and families, and complete detailed home studies. Develop individualized treatment and service plans that address each child’s unique needs. Provide counseling and guidance to children and families throughout placement or discharge. Child and Family Supervision: Oversee children in foster, resource, and adoptive homes, as well as independent living arrangements. Perform regular assessments to determine the child’s needs for medical, psychosocial, educational, and nutritional services. Service Coordination and Continuity: Link children to appropriate services, minimizing barriers to care and coordinating with other agencies to ensure comprehensive service access. Assist in arranging referrals for medical, remedial, and dental services as outlined in each child’s treatment plan. Monitor the ongoing progress of each case, evaluating the effectiveness of the care plan and collaborating with involved parties, including family assessment teams. Education and Counseling: Build a supportive, trusting relationship with the child, promoting growth and well-being in alignment with the service plan. Educate children and families on available resources and work closely with team members to prepare comprehensive case plans.
