Embrace TFC strives to ensure all members involved in the child’s life are supported and are receiving the resources they need to be successful.

About the Role

Are you a compassionate and dedicated mental health professional looking to make a meaningful impact? Embrace Foster Care, an ncgCARE partner, is seeking a Foster Care Case Worker to join our team. At Embrace Treatment Foster Care, our mission is to provide foster children with the best chance for a successful future. Through our CARE model, we offer a comprehensive system of support, training, and guidance, empowering foster families to create a nurturing environment for each child’s growth. The Foster Care Case Worker plays a vital role in our mission to deliver coordinated, individualized care to our communities. As a Foster Care Case Worker, you will provide: Case Management and Support: Conduct interviews with children and families, and complete detailed home studies. Develop individualized treatment and service plans that address each child’s unique needs. Provide counseling and guidance to children and families throughout placement or discharge. Child and Family Supervision: Oversee children in foster, resource, and adoptive homes, as well as independent living arrangements. Perform regular assessments to determine the child’s needs for medical, psychosocial, educational, and nutritional services. Service Coordination and Continuity: Link children to appropriate services, minimizing barriers to care and coordinating with other agencies to ensure comprehensive service access. Assist in arranging referrals for medical, remedial, and dental services as outlined in each child’s treatment plan. Monitor the ongoing progress of each case, evaluating the effectiveness of the care plan and collaborating with involved parties, including family assessment teams. Education and Counseling: Build a supportive, trusting relationship with the child, promoting growth and well-being in alignment with the service plan. Educate children and families on available resources and work closely with team members to prepare comprehensive case plans. This description outlines the general scope and level of work for this role. It is not intended to include every duty or responsibility that may be required of employees in this position. Compensation This role is compensated on a salaried basis, providing consistent earnings aligned with the responsibilities and expectations of the position.

Compensation

  • $47,649.66 - $61,186.56 Placement within the range is based on licensure level, education, and experience. What You Can Expect Support foster children and families through case management, counseling, and individualized care plans. Conduct home visits, interviews, and regular assessments to ensure children’s safety and well-being. Coordinate services like medical, educational, and community resources to meet each child’s needs. Build trusting relationships with children and families while helping guide them toward stability and growth. Responsibilities Conduct interviews, home studies, and assessments for children and foster families. Develop and monitor individualized treatment and service plans for each child. Coordinate medical, educational, counseling, and community support services. Maintain regular communication with families, foster homes, and partner agencies to track progress and ensure quality care.
  • QualificationsQualifications:
  • Education/Experience: Master’s or Doctorate in Social Work or a related field (sociology, psychology, special education, or counseling) from an accredited institution with at least one year of experience in providing casework services to children and families. Bachelor’s degree in Social Work or a related field with relevant experience in casework services or at least two years of experience in casework services with a bachelor’s degree in any field.
