- Summary The Family Case Aid for the School-based Early Support (Prevention) Program plays a crucial role in providing additional casework support to the Family Coordinators.
- This position is responsible for assisting with various tasks related to case management, family support, and program coordination to ensure the successful implementation of prevention services within the school community.
- Essential Functions: Assistance with Case Management: Assist Family Coordinators in conducting assessments, developing service plans, and monitoring progress for families enrolled in the prevention program.
- Provide logistical support for case-related activities, such as scheduling appointments, coordinating transportation, and arranging childcare.
- Family Support Services: Assist in facilitating parent/caregiver support groups and workshops, providing logistical support and assistance as needed.
- Help families navigate community resources and access services related to housing, healthcare, education, and other basic needs.
- Documentation and Record-keeping: Maintain accurate and up-to-date case records, progress notes, and documentation in accordance with program requirements and confidentiality standards.
- Assist with data entry, report preparation, and other administrative tasks to support program evaluation and reporting efforts.
- Collaboration and Communication: Work collaboratively with Family Coordinators, school staff, community partners, and other stakeholders to ensure coordinated service delivery and support for families.
- Communicate regularly with Family Coordinators to provide updates on client needs, progress, and any challenges encountered.
Minimum Qualifications
One year of relevant work in child welfare or social services or life experience; OR Successful completion of an Administration for Children’s Services-funded prevention services program or similar program; OR A high school diploma or equivalent. Good interpersonal skills, ability to work with diverse social, economic, and cultural groups. Proficiency in Microsoft Office. Ability to plan and carry out assignments independently.
