Description Job Overview This position is responsible for working closely with the Family Engagement team to ensure children’s attendance is maintained and families are supported to meet program percentage rate. Will assist with in-kind and data entry.

- Assesses strengths, weaknesses, and needs of the family and develops an individual service plan and re-evaluates the families on a regularly scheduled basis.
- Assists the family in developing goals and objectives and interacting accomplishment of those goals.
- Maintains confidentiality of participants records and information.
- Assists Center Director/Parent Educator in monitoring and follow-up of AVANCE’s core services provided by center-based staff, resource staff, outside agencies and individuals.
- Submits referrals to Center Director/Parent Educator for services needed by families.
- Acts as a liaison between families assigned and designated referring entities to whom referred; serves as an advocate or broker to link families to resources and services as needed.
- Complies with all social work data requirements and schedules for assessments, service plans, develops social history profiles, and maintains up-to-date contact documentation.
- Prepares family needs’ assessments utilizing the Participant Case File Maintenance Form, Family Service Plan Form, Case Notes Form and Adult Literacy Information Form in conjunction with other necessary forms as needed.
- Makes the required number of contacts for each family and provides crisis intervention as warranted.
- Works with staff to help facilitate resources within AVANCE programs.
- Supports participant involvement in program activities, attendance, and follow-ups.
- Participates in all meetings, conferences, briefings, and training sessions as required.
- Prepares necessary staffing summaries and periodically provides progress updates to referring agencies, as necessary.
- May transport participants in own personal vehicle, as necessary.
- Performs other duties as assigned.
Requirements
Education Required: Associate degree or a minimum of 60 credit hours from an accredited institution with a concentration in Social Work or related field.
Preferred
Bachelor’s degree in Social Work with certification.
Work Experience Required
One (1) year experience with high-risk families and familiarity with case management system.
- Additional Skills Required Excellent writing, editing and proofreading skills.
- Computer literate with current Office Software.
- Ability to speak Spanish fluently is helpful but not required.
- Spanish and English fluency is required for positions assigned to PCEP.
- Excellent organizational skills that reflect ability to perform and prioritize multiple tasks with attention to detail.
- Strong interpersonal skills and the ability to build relationships with stakeholders, including staff, board members, external partners, and donors.
- Strong written and verbal communication skills.
- Demonstrated ability to achieve high performance goals, meet deadlines and ability to handle multiple tasks with interruptions in a fast-paced environment.
- Highly resourceful team player, with the ability to be extremely effective independently.
- Strong analytical, presentation, verbal and written communication, problem solving and decision-making skills.
- Emotional maturity.
- Must have dependable transportation daily, valid driver’s license, a clean driving record (subject to annual MVR check and policy), current auto liability insurance and insurable by AVANCE, San Antonio, Inc.
