This facility will serve as another opportunity for us at Youth Haven to support the healing of children, adolescents, and their families through empower and hope! At this new facility, we are hoping to find someone who is self-motivated, creative, and ready to serve the community. We are seeking to hire a full-time, Case Manager for our new facility, opening in August of 2026. Position Summary
Provide all case management duties related to authorization and re-authorization for clients in DT services. Including but not limited to CFT, PCP development, SAR submission, etc.
Qualifications
Education Years of Related Experience Minimum Preferred Minimum Preferred High School Graduate / Equivalent 0-1 Professional Certification 1-3 x Technical School / 1 Year College 3-5 Technical School / 2 Years College / Associate’s Degree 5-8 Bachelor’s Degree x 8-10 Master’s Degree 10+ Advanced Licensure Education: Bachelor’s Degree in the Human service filed Work experience: A minimum of two (2) years of documented clinical experience working with the population served is required. Knowledge, Skills & Abilities: Knowledge of community resources; communication skills (written and verbal); background/training/experience, competencies in the use of a personal computer (word processing/excel), e-notes, photocopier and fax machine; organizational abilities; prioritization skills; flexibility; ability to multitask; ability to develop productive relationships; ability to produce in a fast-paced environment, ability to develop responsible decisions and exercise sound judgment. Takes direction well and demonstrates initiative. Travel Requirements: Service is delivered in school setting. Essential Job Duties % of Time 1. Complete all CFT’s, PCP’s and submit authorizations for clients in Day Treatment services. 50% 2. Actively participate in the development of Person-Centered Plans in conjunction with school personnel, social services professionals, assigned client(s) and their families. Complete and maintain up-to-date client records according to YH standards and prepare the necessary reports and file the client documents in a timely and accurate manner. 25% 3. Complete required documentation in a timely fashion. 10% 4. As necessary, be at the sites for CFT meetings, meeting with Site Managers to discuss client progress. 10% 5.
