Job Status

Full-time; Non-Exempt SCHEDULE: Monday–Friday (8am-4:30pm); Flexible for evenings as needed.

Summary

Provide service coordination for infants and toddlers eligible for First Steps Early Intervention services and their families. Responsible for coordinating evaluations, developing Individualized Family Service Plans (IFSPs), ensuring compliance with state and federal regulations, maintaining accurate documentation, and supporting families through access to services and community resources.

Key Responsibilities

· Respond to all referrals within 2 business days and initiate intake procedures · Complete data entry and maintain electronic records in EI Hub (Indiana’s case management system) · Conduct intake, obtain informed consent, and explain procedural safeguards and family rights · Collect required financial and insurance documentation for Cost Participation · Coordinate and ensure completion of evaluations and eligibility determination within 45 days of referral · Facilitate development, review, and implementation of IFSPs in collaboration with families and providers · Coordinate IFSP meetings, annual reviews, and transition conferences · Monitor and coordinate service delivery across providers and funding sources (including Medicaid and CSHCS) · Assist families in accessing appropriate agencies and community-based resources · Maintain detailed and timely documentation, including monthly clinical logs and reports · Support families in transition planning to preschool or other services prior to age three · Obtain necessary medical documentation from healthcare providers · Provide services in home and community settings based on family needs · Maintain cooperative relationships with families, agencies, and community partners · Submit monthly reports and required documentation by established deadlines

Qualifications

& Skills: · Knowledge of First Steps eligibility criteria, due process, and procedural safeguards · Understanding of IFSP development, early intervention regulations, and transition requirements · Knowledge of community resources and support services · Strong organizational, documentation, and data entry skills · Proficiency in word processing, databases, spreadsheets, and tablet/iPad use · Effective written and verbal communication skills · Ability to work independently and as part of a multidisciplinary team · Strong interpersonal skills when working with families and providers

Physical & Work Requirements

· Ability to travel frequently within assigned service areas · Ability to attend home visits and community-based meetings, including occasional evenings · Ability to use standard office equipment and mobile technology · Valid driver’s license, reliable transportation, and proof of insurance required

Certifications & Training Requirements

· Successful completion of Service Coordination training · Obtain Service Coordinator credential within one year of hire · Maintain ongoing training and credential requirements
